Student Extension Request Form

The Maximum Time Limit on the Register as an Accredited Student Sonographer policy should be read prior to completing this form.

To apply for an extension, a Student Sonographer must provide supporting evidence to ASAR, explaining the reasons for their request. Please complete the below form. The information will be presented to the ASAR Board during their next available meeting. The ASAR Board convenes in February, April, July, September, and November each year. The Student Sonographer will receive a response regarding their request within 7 days after the ASAR Board’s meeting.
