ASAR forms
The Commonwealth Department of Health and Aged Care has proposed regulations for Medicare ultrasound benefits effective from the 1st of November 2001. From this day sonographers performing ultrasound examinations on behalf of medical practitioners will be required to be accredited. This requirement will become part of the Medicare Benefits Schedule regulations. A register of qualified and accredited sonographers will be held by the Health Insurance Commission.
Application for entry to the Register of Accredited Medical Sonographers
- This form is to be completed by applicants who wish to apply for entry into the Register of Accredited Medical Sonographers .
Application for entry to the Register of Accredited Student Sonographers
- This form is to be completed by applicants who wish to apply for entry into the Register of Accredited Student Sonographers.
Application for deactivation from the Register
- This form is to be completed by registrants who wish to be teporarily de-activated from the Register due to maternity leave, travel, or any other reason. Please note that ASAR will not refund any money already paid, however no further payment will be charged during the de-activation period. Registrants must complete an ASAR Re-activation form when they wish to be re-instated to the Register.
Application for reactivation to the Register
- This form is to be completed by registrants who wish to be re-activated to the ASAR Register after obtaining a temporary de-activation.
ASAR Decision Appeal Form
- This form is to be used by any person who wishes to appeal against a
decision of the Registry. The form needs to be lodged within thirty days
after the receipt of communication of that decision and accompanied by an
application fee as prescribed in the form.
CPD: Supervisor's Validation Form
- The ASAR recognises that not all CPD activities include a certificate of attendance; for example, grand rounds and in-house seminars. Documentation of attendance at these meetings may include a signature from your supervisor on this form.
CPD: Self-Directed Learning Activity Statement
- A Self-Directed Learning Activity Statement is evidence of self-directed learning activity. A learning outcome is a statement, which reflects additional knowledge gained from the activity, which may be used to change or reinforce your current clinical practice
CPD: Activity Report
- The CPD Activity Report is the only documentation that should be regularly submitted to ASAR. A CPD activity report is the method of logging CPD activities onto your personal ASAR CPD database. A CPD activity report proforma will be provided for you to complete each year with your annual subscription notice. Activity reports may be submitted at any time during the year and may also be completed on line via the ASAR website. Full instructions, including category codes, are available in the CPD page. You may also submit CPD Activity Reports via the ASAR website.
CPD: Credit Allocation Form
- Wherever possible the meeting / program provider is to calculate the CPD credits using the guide in Table 1. If this is not possible a Request for a CPD Credit Allocation form is to be submitted to the secretariat for allocation of points.CPD credit allocation is based on the duration, level of involvement and type of program.The co-ordinator / organiser may apply for a whole years program in advance. Application may be made for programs retrospectively but prospective applications are desirable.
CPD: Certificate of Attendance
- It is desirable for co-ordinators/organisers to provide attendees with a certificate of attendance. This certificate should clearly state meeting/program title, convenor's name, date, participant's name (may be handwritten), allocated CPD Credits, ASAR Activity Code, and Postcode of location.
Please complete printed versions of these forms with a typewriter or print with a black ball-point pen, and then post to ASAR. Only certified copies of sonography qualifications and name changes will be accepted. Incomplete or inaccurate forms will be returned.
